As part of the additional £8m health budget awarded to Lincolnshire, local non profit project PACT, Lincolnshire Partnership Foundation Trust (LPFT) and Lincoln Hospital Trust (LHT) are joining forces.
A substantial amount of this budget has been directed towards preparing for ‘winter pressures’, following last years weather induced A&E closures, cancelled operations, bed pressures & ambulance delays, as well as the increased demand for healthcare caused by the cold. LPFT & LHT are leading the way with a ‘joined up commissioning’ approach; using a single provider consortium (PACT) to reduce the spectrum of anticipated healthcare pressures.
For some this might mean;
– Ensuring they get access to their home
– That the heating is on
– That they understand their new medication
– That other social care and medical visitors have the correct information for seamless care
– Communicating with neighbours and family
– Eradicating any obvious dangers i.e. the reason for a fall which caused hospital admission in the first place
– Making sure they have enough fresh groceries in the home
– That they have their day-to day belongings in the ‘right place’ (spectacles, commode, walking stick or frame, Telecare system etc).

PACT is a project led by Adults Supporting Adults (ASA) joined by Lace Housing Association, Lincolnshire Home Improvement Agency, and AGE UK Lincoln. The project so far has featured at Lincoln Community Hospital and has already forecasted savings of £500,000 up to December end, having supported a whopping 800 clients with transport and care-in-the home-services.
PACT is soon to feature at Grantham and Boston Hospitals before the end of the year and will form as a support to East Midlands Ambulance Service this year also.
The budget is allocated for Winter 2013/2014, ending at the end of March. The project will however provide a solid foundation to justify further funding and will hopefully become integrated and sustainable throughout the year, every year.
